This is a revision rhinoplasty case involving a 23-year-old woman who had already undergone two prior septorhinoplasties and came in unhappy with persistent nasal deviation. Because previous surgeries had depleted usable cartilage, a rib graft was used to provide the structural support needed to address the deviation. Nasal valve repair was performed alongside resection of the inferior turbinates. The after photo, taken 13 months post-op, shows the nose sitting straighter than it did before surgery. Cases like this one, where scar tissue and prior structural changes complicate the anatomy, require a different planning process from that for a primary rhinoplasty.
